To receive a payment after the Court grants final approval, complete the applicable forms from the Forms to Receive Payment section and collect the verification documents requested in the forms. Living Class Members should complete the Class Member Verification Form. Those with authority to collect the payment of a deceased Class Member should complete the Successor Verification Form.
You can email the completed verification form and related documents to the Settlement Administrator at the email address listed on the form. Alternatively, you can use the Online Portal to complete the form and upload the related documents through this website.
There is currently no deadline to submit the verification forms and related documents. Deadlines will be set and posted here when the Court grants final approval to the settlement following the November 3, 2026 hearing. But you can still submit all forms and documents now to expedite processing.
